Annie Hazel McDade
Winston-Salem - Mrs. Hazel McDade Lewis, 88, formerly of Orange County, NC, died Saturday, January 6, 2001, at the Triad United Methodist Home, Winston-Salem. She was born August 15, 1912, to the late John C and Ellie McAdams McDade of Orange County.
Mrs. Lewis was a graduate of Appalachian State University and taught in Orange County and Lexington schools. After moving to Winston-Salem, she became a member of the Ardmore United Methodist Church where she was active in the Alma Nunn Class and the Women's Society of Christian Service.
She was preceded in death by her husband, Lee Craig Lewis; and her son, Carlos Edwin Lewis. Survivors include one son, Lee Craig Lewis, Jr and wife, Carole of Waxhaw; one grandson Andrew Craig Lewis and wife, Angela of Monroe; tow great-grandchildren; one sister, Mary Ruth Kaiser and husband Richard, of Paola, Kan; three nieces, Anne Evans, of Lawrence, Kan, Susan Young of Laplata, MD and Joe Westbrook of Efland, NC and one nephew, Frank P McDade of Hillsborough.
Funeral services will be conducted Tuesday at 10:00AM at Vogler & Sons Main Street Chapel in Winston-Salem by the Rev Lisa Wishon. A graveside service will follow at 2:00PM at Greenwood Cemetery in Blemont. The family will receive friends Monday from 7:00 until 9:00PM at the funeral home.
